online, save the information to your desktop, and then print the completed
form for submission to the Plans. We hope that you will find the time savings

Updated Multiple Service Contract Policies and Procedures


According to the Screen Actors Guild 2009 Commercials Contract, all signatories
are required to designate multiple-service contract status on the contribution
remittance reports filed with the Plans when contributions are tendered in
connection with services related to a multiple-service agreement. Signatories
are also required to provide unredacted copies of all contracts relating to
services provided under such multiple-service agreements at the time of
submission of initial contribution reports to the Plans, except in instances
where the Plans agree to examine the underlying agreement in Los Angeles or
New York. Due to the confidential and sensitive nature of the terms of these
performance agreements, the Plans take all necessary steps to ensure the
confidentiality of these agreements throughout our process.

Please be advised that the Trust Agreements provide that the failure to supply
the requested data described by the report subjects the producer to liquidated
damages of 10% of the contribution amount for each performer, and if the failure
to supply the data persists for more than 60 days from the due date, the
liquidated damages increase to 20% of the contribution amount. Liquidated
damages are mandatory penalties provided by the Trust Agreements and ERISA.

Revised Industrial and Educational Contribution Reporting Form


Please be advised that effective November 1, 2009, the Screen Actors Guild - Producers Pension and Health Plans have revised the Industrial and Educational Contribution Reporting form to incorporate the increase in contribution rate from 14.8% to 15.3% for Pension and Health for submitted contributions according to the 2009 Extension to the 2005 Screen Actors Guild Industrial & Educational Contract. Revised Television/Theatrical Contribution Form


Please be advised that effective June 10, 2009, the contribution rate for Principal
Performers is 15.3% and Background Performers is 15.0%. Such contributions are
allocated as follows: .3% to the "Industry Advancement Cooperative Fund" (IACF) and
15% to the SAG - Producers Pension and Health Plans.

For any pilot or presentation produced on or after June 10, 2009 and for the first two
seasons of any new one-hour series which is first exhibited in the 2009-10 television
season, regardless of the fact that a pilot or one or more episodes of the series may
have been produced prior to June 10, 2009, the pension and health contribution rate
for performers and background performers will be 13.5% of compensation and there
shall be no contributions to the IACF.

Revised TV Commercials - Exhibit B Contribution Form


Please be advised that effective April 1, 2009, the Screen Actors Guild - Producers
Pension and Health Plans have revised the TV Commercial - Exhibit B Contribution
Reporting Form to incorporate the increase in contribution rate from 14.8% to 15.5%
for Pension and Health for submitted contributions according to the 2009 Screen
Actors Guild Commercials Contract. The 15.5% rate applies to principal photography
produced on or after April 1, 2009. Please click here for the current form.

In addition, Effective April 1, 2009, Section 46 E-1 of the SAG Commercials Contract
has been amended to reflect the following:

Producer shall designate multi-service contract status on the contribution remittance
reports filed with the Plans when contributions are tendered in connection with
services related to multiple-service agreement. Producer agrees to provide
unredacted copies of all contracts relating to services provided under such
multiple-service agreements to SAG and to the Plans at the time of submission
of initial contribution reports to the Plans.

A multiple-service agreement should be noted with a check mark in column (10) on
page 1 of the TV Commercials Exhibit B Contribution Report, and the Performer
Category column (3) should be identified with the MSC Performer Category Code.
In addition, please submit a copy of the multiple-service agreement with the
submission of contributions to the Plans.
